This page provides links to forms which you can download, fill in and file in your case in the Family Courts.  The Family Courts only exist in Rockingham and Grafton Counties of this State.  Therefore, only use these forms if you live in either Rockingham or Grafton Counties.  If you live in any other County use the Superior Court Forms.

INDIVIDUAL PETITION FOR DIVORCE:  Use this form in an individual wants to commence a divorce action.  Typically this form is used for irreconcilable or no-fault divorces but it can also be used to commence a fault based divorce.  If you would like to file on fault grounds you may want to strongly consider consulting with the Divorce Law Group so you are clear on your rights and obligations in a fault based divorce.  MS WORD or PDF formats.
