PETITION FOR SEPARATION:
Use this form if you only want a legal separation and not a
divorce. MS

JOINT PETITION FOR DIVORCE: Use this form if both a couple want to commence a
divorce together in the hope they can work together to resolve
whatever issues they may have and complete their divorce in a
joint fashion. MS

INDIVIDUAL PETITION FOR DIVORCE:
Use this form in an individual wants to commence a divorce
action. Typically this form is used for irreconcilable or
no-fault divorces but it can also be used to commence a fault
based divorce. If you would like to file on fault grounds
you may want to strongly consider consulting with the Divorce
Law Group so you are clear on your rights and obligations in a

AFFIDAVIT FOR DEFAULT CASE: Use
this form when the person you have filed a divorce against has
failed to provide you with a Financial Affidavit to file with
the Court and, despite your best efforts, you are unable to
obtain it. MS

PETITION TO MODIFY COURT ORDER: Use this form if you want to change any
order of the Court. You simply explain why you want to
change the current order of the Court and what you want the new

PETITION TO MODIFY CHILD SUPPORT ORDER: Use this form if you want to change
an existing child support order. This is a specialized
version of the Petition To Modify Court Order listed

PETITION FOR CUSTODY AND CHILD SUPPORT: Use this form if you want to obtain
an order regarding custody of the children and for child
support. MS

PETITION FOR CONTEMPT:
Use this form if there has been a violation of an order of the
Court which you want the Court to consider the violation and
enter an order of contempt against the other Party. MS

GENERAL PURPOSE MOTION:
This is a general purpose motion form. Use it whenever you
would like the Court to enter an order about some issue that is
not covered by some other form in this list. MS

PERSONAL DATA SHEET:
Fill this form out and file it with the Court so the Court has
your personal information. It is generally filed at the
start of the divorce case. If you do not want your spouse
to know where you live due to domestic violence reasons, you can
instruct the Court to not release your home address to your

PETITION FOR PATERNITY:
Use this for if you want the Court to establish paternity of a
